Our editorial standard places a cream in Avoid when the cited testing reports detectable lead, cadmium, mercury, or arsenic. This is a precautionary purchasing standard, not a diagnosis, a legal finding, or proof that normal use causes harm. A Clean label means the evidence meets this standard; it is not a guarantee of zero contaminants or suitability for every child.

The reports below describe particular samples. Non-detect means below the method’s reporting limit, not zero. These tests do not measure how much a child absorbs, compare rash-treatment effectiveness, or establish the results for every future batch.

Zinc oxide and petrolatum remain barrier ingredients discussed in the American Academy of Pediatrics’ diaper-rash guidance. A finished-product finding does not establish which ingredient introduced a metal.

Ingredient concerns for diaper-rash creams

  • Zinc oxideUse with cautionUse with caution, and check the specific product before assuming "zinc oxide" alone tells you anything. Pharmaceutical- and cosmetic-grade zinc oxide from a reputable manufacturer can meet purity specifications (typically USP or equivalent) that cap heavy metal content at trace levels — but independent testing across 2025-2026 found detectable lead in nearly every zinc-oxide diaper-rash cream tested regardless of price or "clean" marketing, with several results exceeding Washington State's new cosmetic lead limit of 1 part per million (1,000 ppb, effective January 2025) by three to four times over. FDA's own cosmetic lead guidance — a non-binding 2016 draft recommendation, not an enforceable limit — puts a recommended maximum around 10 ppm (10,000 ppb), so these results sit well under federal guidance while still exceeding the stricter state standard; that gap is exactly why "not illegal" and "not detected" are different claims, and why a brand can legally sell a product that independent testing flags as concerning. One non-zinc alternative (a plain petrolatum-based balm, tested by the same initiative) came back non-detect for lead, cadmium, mercury, and arsenic — proof the contamination isn't inherent to treating diaper rash, only to this specific supply chain. Label phrases that signal a problem

  • Match the product name and zinc-oxide percentage to the tested version. Similar packaging can describe a different formula.
  • Non-detect means below a test’s reporting limit. It does not mean zero.
  • A certification, an ingredient list and a laboratory report answer different questions.

Frequently asked questions

Does a detected metal mean a product is illegal or recalled?
No such conclusion follows from the test alone. Product classification, applicable rules and enforcement policies matter. Our Avoid label describes our purchasing standard, not a regulatory decision. See Washington’s current guidance.
Does this mean zinc oxide should never be used?
No. The American Academy of Pediatrics discusses zinc oxide and petrolatum as barrier options in its diaper-rash guidance. These reports concern tested finished products, and do not prove that zinc oxide caused a finding or that all zinc-oxide creams have the same results.
Can I compare the cream results directly with diaper or wipe tests?
Not as a ranking of health risk. They involve different substances, methods, reporting limits and exposure routes. A concentration in a product is not a measurement of what a child absorbs.
What should I check before buying?
Match the exact formula and active-ingredient percentage, read the report date and limits, and check the current label. If your child has a persistent rash or reacts to a product, ask your pediatrician about treatment rather than relying on a product ranking.
