Cocamidopropyl Betaine
What it is
Cocamidopropyl betaine (CAPB) is a synthetic, zwitterionic (amphoteric) surfactant manufactured by reacting fatty acids derived from coconut oil with a chemical called dimethylaminopropylamine (DMAPA). Despite the "coconut-derived" marketing language it's often given, the finished ingredient is a processed derivative several synthesis steps removed from raw coconut oil, not the oil itself. It's prized in baby shampoos, bubble baths, and "tear-free" cleansers as a foam booster and co-surfactant that lets formulators use less of the harsher primary detergents (like sodium lauryl sulfate), producing a milder, lower-irritation wash than a straight detergent alone.
Why it's flagged
The dermatology literature on CAPB has converged on a specific mechanism: the allergic contact dermatitis associated with this ingredient is driven mainly by residual synthesis byproducts — DMAPA and a related impurity called amidoamine (AA) — left over in commercial-grade CAPB, rather than by the purified betaine molecule itself. Fowler's 1997 patch-test study (Contact Dermatitis) found that reactions tracked with amidoamine content, and that CAPB purified to remove amidoamine did not provoke reactions in previously sensitized patients. Sensitization rates climbed through the 1990s and early 2000s, and North American Contact Dermatitis Group patch-test data (Fowler et al., 2004, Dermatitis) led the American Contact Dermatitis Society to name CAPB its 2004 "Allergen of the Year." The Cosmetic Ingredient Review (CIR) Expert Panel's 2012 final safety assessment reached a matching conclusion: CAPB is safe as used, provided it is manufactured and formulated to be non-sensitizing — meaning amidoamine/DMAPA impurities are controlled below a low threshold.
How concerned to be
This is a genuine case for context-dependent rather than blanket concern, and the newer research actually tempers the alarm of the "Allergen of the Year" headline. A 2011 analysis of German patch-test network data spanning 1996–2009 (Schnuch et al., Contact Dermatitis) found that true allergic reactions to CAPB are rare across the general tested population, consistent with CIR's view that well-manufactured, low-impurity CAPB is not a significant sensitizer for most users. The real-world risk concentrates in a smaller group: people with eczema, atopic dermatitis, or an already-compromised skin barrier, where residual impurity has an easier path to trigger a reaction — which is exactly the population a baby-products buyer is often trying to protect, and the concern the original draft correctly flagged. The genuine unresolved gap is that amidoamine/DMAPA impurity levels vary by manufacturer and batch and are never disclosed on a product label, so there's no way to tell a cleaner CAPB batch from a higher-impurity one at the shelf — a real data gap, not a documented hazard for the ingredient class as a whole. Practical takeaway: for most babies this is a low-risk, well-studied mildness ingredient; for a baby with eczema or broken skin, patch-test a new CAPB-containing product and discontinue if irritation appears.
